Objetivos: Evaluar el papel del radiofarmacéutico en un equipo multidisciplinar en la detección de contraindicaciones del regadenosón para su uso seguro en pacientes a los que se solicitó una SPECT de perfusión miocárdica. Métodos: Se estudió ambispectivamente su uso seguro en 1.905 pacientes (54,1% mujeres, edad media: 66,6±11,7 años, rango: 20-95años). Se registraron datos relativos al sexo, a la edad, al historial médico, a la medicación, a las alergias medicamentosas y a las contraindicaciones para el estrés farmacológico, así como las recomendaciones realizadas al médico nuclear responsable. Resultados: Las contraindicaciones detectadas y las correspondientes recomendaciones fueron las siguientes: riesgo de prolongación del intervalo QTc (7,5%): comprobación previa del intervalo QTc y monitorización del ECG; ictus o AIT previo (4,2%): evaluación de estenosis carotídea; alergia a salicilatos y/o sulfamidas (3,1%): empleo de [99mTc]Tc-MIBI; epilepsia o riesgo de convulsiones (2,4%): uso de adenosina o reconsiderar su indicación; tratamiento con corticosteroides sistémicos en EPOC severa (1,3%): reevaluar las condiciones del paciente; EPOC reagudizada (0,8%): posponer hasta la resolución del episodio agudo; asma grave (0,4%): no realizar la prueba; toma de metilxantinas (0,3%): evitar su consumo previo; otras (6,1%): evaluación de cada contraindicación. No se observaron contraindicaciones en el 73,6% de los pacientes. Se anularon el 2,9% de las peticiones debido a contraindicaciones absolutas. Conclusiones: Empleando una metodología de trabajo sistemática, el radiofarmacéutico detectó un elevado número de incidencias, presentando uno de cada cuatro pacientes alguna contraindicación clínica. Las recomendaciones emitidas fueron aceptadas por los médicos nucleares, que modificaron su enfoque, incrementando así la seguridad de estos pacientes.(AU)

Results: Detected contraindications and corresponding recommendations were as follows: risk factors for QTc interval prolongation 7.5% — measurement of QTc interval previously to test and monitor ECG; prior stroke or TIA 4.2% — consider carotid stenosis assessment; salicylates/sulfonamides allergy 3.1% — use 99mTc-sestamibi; epilepsy or risk factors for seizures 2.4% — use of adenosine or reconsider test indication; systemic corticosteroid therapy for severe COPD 1.3% — reassessment of patient's condition; acute exacerbation of COPD 0.8% — defer test until acute episode is over; severe asthma 0.4% — do not perform test; methylxanthine ingestion 0.3% — avoid consumption previously; other 6.1% — evaluation of other contraindications. No contraindications were detected in 73.6% of patients. The test was cancelled due to absolute contraindications in 2.9% of the requests. Conclusions: Working in a systematic way, the radiopharmacist was able to detect a high number of issues related to regadenoson, with one out of four patients presenting some clinical contraindication. The recommendations given by the radiopharmacist were well accepted by the nuclear physicians who changed their approach contributing to increase the safety of patients referred for MPI.(AU)

Introduction: Recent progress in the bioengineering of cardiac grafts offers a new therapeutic modality for the regeneration of cardiac tissue after myocardial infarction. CD34 is a marker that expresses all hematopoietic and endothelial precursor cells, and functions as a cell adhesion factor. The antibody corresponding to this marker is used in immunohistochemistry to evaluate the formation of new vessels and the presence of stem cells. Objective: To evaluate the effectiveness of omentopexy in neovascularization and stem cells. Methods: Review collecting information published on selected virtual platforms (SciELO ­ Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus). The search began with descriptors related to the topic, identified through DeCS/MESH in the following: "myocardial ischemia; myocardial revascularization; omentopexy; immunohistochemistry; CD34; stem cells". They were read by title and summary, with an AND or OR search followed by the full reading of those most closely related to the topic. A total of 23 articles were included. Results: The greater omentum has a remarkable clinical property in containing sites of tissue damage. It increases its tissue volume in response to foreign and inflammatory particles involving several immunomodulatory cells with progenitor cells, in a process called "omentum activation". Conclusion: Cardio-omentopexy associated with mechanical abrasion and myocardial perforations proves to be efficient in inducing neovascularization. The greater omentum promotes stem cells - confirmed by CD34 -, demonstrating great potential as a future therapy to restore areas of ischemic myocardium.


Introdução: Recentes progressos feitos na bioengenharia de enxertos cardíacos oferecem nova modalidade terapêutica para a regeneração do tecido cardíaco pós-infarto do miocárdio. O CD34 é marcador que expressa todas as células precursoras hematopoiéticas e endoteliais, e funciona como fator de adesão celular. O anticorpo que correspondente a este marcador é utilizado na imunoistoquímica para avaliar a formação de novos vasos e a presença de células-tronco. Objetivo: Avaliar a eficácia da omentopexia na neovascularização e células-tronco. Métodos:  Revisão colhendo informações publicadas em plataformas virtuais selecionadas (SciELO ­ Scientific Electronic Library Online, Google Scholar, Pubmed e Scopus).  A busca iniciou-se por descritores relacionados ao tema, identificados por meio do DeCS/MESH nos seguintes descritores: "isquemia miocárdica; revascularização miocárdica; omentopexia; imunoistoquímica; CD34; células-tronco" e seus equivalentes em inglês: "stem cell; myocardial ischemia; myocardial revascularization; omentopexy; immunohistochemistry; CD34". Foram lidos pelo título e resumo, com busca AND ou OR seguindo-se da leitura na íntegra daqueles com maior relação ao tema. Foram incluídos o total de 23 artigos. Resultados: O omento maior tem propriedade clínica marcante em conter sítios de danos teciduais. Ele aumenta seu volume tecidual em resposta às partículas estranhas e inflamatórias envolvendo diversas células imunomoduladoras com células progenitoras, em um processo chamado "ativação do omento". Conclusão: A cárdio-omentopexia associada à abrasão mecânica e perfurações miocárdicas, mostra ser eficiente na indução de neovascularização. O omento maior promove células-tronco - confirmadas pelo CD34 -, demonstrando grande potencial como futura terapêutica para restaurar áreas de miocárdio isquêmico.

Introducción El estudio diagnóstico no invasivo del riesgo cardiovascular en pacientes que van a ser llevados a trasplante hepático no es claro especialmente en asintomáticos. Respecto a la gammagrafía de perfusión miocárdica (GPM) se ha pensado que la reserva vasodilatadora deteriorada en estos pacientes puede reducir su rendimiento. El objetivo es valorar el papel de la GPM en la evaluación prequirúrgica de los pacientes que van a ser tratados mediante un trasplante hepático. Material y métodos Estudio retrospectivo, descriptivo y observacional. Se incluyó a todos los pacientes adultos llevados a trasplante hepático entre 2017 y 2021 que tuvieran GPM previa. Se describen los hallazgos de la GPM y se correlacionaron con los hallazgos de angiografía invasiva y con la aparición o no de eventos cardiovasculares peri y postrasplante. Resultados De 188 pacientes trasplantados (edad promedio: 57 años, DE: 12), 178 tenían perfusión miocárdica previa, 82 (46%) pacientes no tenían factores de riesgo cardiovascular y 5 (2,8%) tenían antecedente de enfermedad coronaria. De las GPM, 177 fueron con estrés con dipiridamol realizadas en promedio 10 meses antes del trasplante. Únicamente 17/178 (9,5%) estudios fueron anormales. El seguimiento medio fue de 38 meses (DE: 10). De los pacientes con GPM normal, solo 2 (1,2%) presentaron eventos cardiovasculares, ambos con estudios realizados más de 2años atrás. No hubo muertes de origen cardiovascular. Conclusiones La GPM es una técnica segura y confiable en la valoración cardiovascular en pacientes candidatos a trasplante hepático dada la baja tasa de falsos negativos en el seguimiento (AU)

Of the patients with normal MPS, only 2 (1.2%) presented cardiovascular events, both with studies performed more than 2years before the procedure. There were no deaths of cardiovascular origin. Conclusions MPS is a safe and reliable technique for cardiovascular assessment of patients who are candidates for liver transplantation, given the low rate of false negatives during follow-up (AU)

Antecedentes. El valor pronóstico de una ergometría positiva en el contexto de imágenes tomográficas de perfusión miocárdica de estrés y reposo (SPECT) normales no está bien establecido. Objetivos. Documentar la incidencia de infarto, muerte y revascularización coronaria en pacientes con una ergometría positiva de riesgo intermedio e imágenes de perfusión SPECT normales, y explorar el potencial valor del puntaje de riesgo de Framingham en la estratificación pronóstica de estos pacientes. Métodos. Cohorte retrospectiva integrada por pacientes que habían presentado síntomas o hallazgos electrocardiográficos compatibles con enfermedad arterial coronaria durante la prueba de esfuerzo, con criterios de riesgo intermedio en la puntuación de Duke y perfusión miocárdica SPECT normal. Fueron identificados a partir de la base de datos del laboratorio de cardiología nuclear del Instituto de Cardiología y Cirugía Cardiovascular de la ciudad de Posadas, Argentina. Resultados. Fueron elegibles 217 pacientes. El seguimiento fue de 3 1,5 años. La sobrevida libre de eventos (muerte,infarto de miocardio no fatal, angioplastia coronaria o cirugía de bypass de arteria coronaria) a uno, tres y cinco años fue significativamente menor (Log-rank test, p= 0,001) en el grupo con puntaje de Framingham alto o muy alto (77, 71y 59 %, respectivamente) que en el grupo de puntaje bajo o intermedio (89, 87 y 83 %). Tomando como referencia a los pacientes con riesgo bajo en el puntaje de Framingham, luego de ajustar por edad, sexo y puntaje de Duke, los pacientes categorizados en los estratos alto y muy alto riesgo del puntaje de Framingham presentaron una incidencia del evento combinado cercana al triple (hazard ratio [HR] 2,81; intervalo de confianza [IC] del 95 % 0,91 a 8,72; p= 0,07 y HR 3,61;IC 95 % 1,23 a 10,56; p= 0,019 respectivamente). Conclusiones. La estimación de riesgo con el puntaje de Framingham sería de ayuda en la estratificación pronóstica de los pacientes con ergometría positiva y SPECT normal. (AU)


Background. The prognostic value of positive exercise testing with normal SPECT myocardial perfusion imaging is not well established. Objectives. To document the incidence of infarction, death, and coronary revascularization in patients with a positive intermediate-risk exercise test and normal SPECT perfusion images and to explore the potential value of the Framingham Risk Score in the prognostic stratification of these patients. Methods. A retrospective cohort comprised patients who presented symptoms or electrocardiographic findings compatible with coronary artery disease during the stress test, with intermediate risk criteria in the Duke score and normal SPECT myocardial perfusion. They were identified from the database of the nuclear cardiology laboratory of the Instituto de Cardiología y Cirugía Cardiovascular of Posadas, Argentina. Results. 217 patients were eligible. Follow-up was 3 1.5 years. Event-free survival (death, non-fatal myocardial infarction, coronary angioplasty, or coronary artery bypass surgery) at one, three, and five years was significantly lower (Log-ranktest, p: 0.001) in the group with a score of Framingham high or very high (77, 71 and 59 %, respectively) than in the lowor intermediate score group (89, 87 and 83 %). Taking as reference the low-risk patients in the Framingham score, after adjusting for age, sex, and Duke score, the patients categorized in the high-risk and very high-risk strata showed about three times higher incidence of the combined event (hazard ratio [HR] 2.81; 95 % confidence interval [CI] 0.91 to 8.72;p=0.07 and HR 3.61; 95 % CI 1.23 to 10.56; p=0.019 respectively). Conclusions. Risk estimation with the Framingham score would be helpful in the prognostic stratification of patients with positive exercise testing and normal SPECT. (AU)

La evaluación de la perfusión miocárdica con SPECT combina una prueba de esfuerzo (ergometría o estrés farmacológico) junto a imágenes de perfusión con radioisótopos. Este estudio es útil para establecer el diagnóstico de enfermedad arterial coronaria, estratificar el riesgo de infarto y tomar decisiones terapéuticas. Un resultado normal aporta un alto valor predictivo negativo, es decir, una muy baja probabilidad de que el paciente presente eventos cardiovasculares. El hallazgo de signos de isquemia en la ergometría podría poner en jaque el valor predictivo negativo de una perfusión normal. En presencia de este resultado, el paso siguiente es evaluar los predictores de riesgo en la ergometría, el riesgo propio del paciente en función de los antecedentes clínicos y el puntaje cálcico coronario, cuando este se encuentra disponible. Ante la presencia concomitante de otros marcadores de riesgo se sugiere completar la evaluación con un estudio anatómico.El uso de nuevas tecnologías podría mejorar la precisión en la predicción de eventos. Assessment of myocardial perfusion with SPECT combines a stress test (ergometry or pharmacological stress) with radioisotope perfusion imaging. This test is helpful to diagnose coronary artery disease, stratify the risk of heart attack, and make therapeutic decisions. A normal result provides a high negative predictive value; therefore, the probability of cardiovascular events is very low. Signs of ischemia on an ergometry could jeopardize the negative predictive value of normal perfusion. In this clinical setting, the next step is to evaluate the risk predictors in the stress test, the individual risk based on the clinical history, and the coronary calcium score when available. Given the simultaneous presence of other risk markers,completing the evaluation with an anatomical study is suggested. The use of new technologies could improve the accuracy of event prediction. The objective is to describe home patient care plans after myocardial revascularization (MR) based on the Mid-Range Nursing Theory for Cardiovascular Rehabilitation (TMA Enf-RCV). This is a multiple case study in which data collection was performed at the home of seven patients in the postoperative period of MRI, in Sobral-Ceará, between June and December 2019. The information collected was organized in individual reports and then, joint analytical synthesis was performed. The assessment of rehabilitating behavior and stimulus for cardiovascular rehabilitation (CVR) involved physiological adaptive problems, self-concept, role performance and interdependence, with 50% of nursing diagnoses focusing on problems and being associated with the physiological mode and57% of health promotion diagnoses were related to self-concept mode. The nursing rehabilitation intervention included the promotion of physical exercises, encouragement of the patient and family in care, education about adaptive strategies, psychosocial support, among others. This study supports the applicability of TMA Enf-RCV as an effective intervention for CVR with a focus on quality of life.(AU)


El objetivo es describir los planes de atención domiciliaria del paciente después de la revascularización miocárdica (RM) basados en la Teoría de Enfermería de Rango Medio para la Rehabilitación Cardiovascular (TMA Enf-RCV). Se trata de un estudio de caso múltiple en el que la recogida de datos se realizó en el domicilio de siete pacientes en el postoperatorio de RM, en Sobral-Ceará, entre junio y diciembre de 2019. La información recogida se organizó en informes individuales y luego, se realizó la síntesis analítica conjunta. La evaluación de la conducta rehabilitadora y el estímulo para la rehabilitación cardiovascular (RCV) involucró problemas fisiológicos adaptativos, autoconcepto, desempeño de roles e interdependencia, con un 50% de los diagnósticos de enfermería centrados en problemas y asociados con el modo fisiológico y un 57% de los diagnósticos de promoción de la salud se relacionaron con el modo de autoconcepto. La intervención de rehabilitación de enfermería incluyó la promoción de ejercicios físicos, el estímulo del paciente y la familia en el cuidado, educación sobre estrategias adaptativas, apoyo psicosocial, entre otros. Este estudio respalda la aplicabilidad de TMA Enf-RCV como una intervención eficaz para el RCV con un enfoque en la calidad de vida.(AU)


Objetivase descrever planos de cuidados de pacientes em domicílio após revascularização miocárdica (RM) fundamentados na Teoria de Enfermagem de Médio Alcance para Reabilitação Cardiovascular (TMA Enf-RCV). Tratase de um estudo de casos múltiplo no qual a coleta de dados realizouse no domicílio de sete pacientes em pós-operatório de RM, em Sobral-Ceará, entre junho e dezembro de 2019. As informações coletadas foram organizadas em relatórios individuais e em seguida, realizada síntese analítica conjunta. A avaliação do comportamento reabilitador e estímulo para reabilitação cardiovascular (RCV) envolveram pro-blemas adaptativos fisiológicos, de autoconceito, de desempenho de papel e de interdependência, sendo que 50% dos diagnósticos de enfermagem tinham foco nos problemas e se associaram ao modo fisiológico e 57%dos diagnósticos de promoção da saúde foram referentes ao modo autoconceito. A intervenção reabilitadora de enfermagem contemplou promoção de exercícios físicos, encorajamento do paciente e da família no cuidado, educação acerca de estratégias adaptativas, suporte psicossocial, dentre outros. Este estudo subsidia a aplicabilidade da TMA Enf-RCV como intervenção efetiva para a RCV com foco na qualidade de vida.(AU)

RESUMEN La decisión sobre la mejor estrategia de revascularización para los pacientes con enfermedad de múltiples vasos se ha tornado una tarea compleja a medida que la angioplastia coronaria ha mejorado sus resultados. En la siguiente revisión nos propusimos evaluar las variables que en nuestra experiencia definen el beneficio de una técnica sobre la otra, entendiendo que de esta manera la decisión del médico tratante se hace más sencilla y objetiva. Por otro lado, y festejando el saludable protagonismo que se le da al paciente, creemos que esta evaluación permite ofrecer argumentos sólidos para ayudarlo en la toma de la decisión.


ABSTRACT The decision on the best revascularization strategy for patients with multivessel disease has become a complex task as coronary angioplasty has improved its results. In the following review, we set out to evaluate the variables that, in our experience, define the benefit of one technique over the other, understanding that in this way the treating physician's decision will become simpler and more objective. On the other hand, and celebrating the healthy prominence given to patients, we believe that this evaluation allows solid arguments to help them in decision making.

Introducción: La enfermedad por coronavirus ha causado daño miocárdico, razón que ha impactado en las ciencias médicas por lo que ha sido motivo de investigación. Objetivo: Mostrar a través de resultados de recientes investigaciones, los mecanismos mediante los cuales el virus SARS-CoV-2 produce daño miocárdico en los pacientes afectados por la COVID-19, y su influencia en el pronóstico a corto y largo plazo. Métodos: Se realizó una búsqueda bibliográfica de la literatura médica actualizada sobre el tema publicada en idioma inglés y español, indexado en varias bases de datos en el período comprendido de mayo de 2019 a mayo de 2022. De un total de 198 artículos en la revisión, cumplieron con los criterios de selección 78 artículos. Se excluyeron los que no contenían información precisa en cuanto al daño miocárdico provocado por el SARS-CoV-2. Resultados: Se han descrito varios mecanismos que pueden ser los desencadenantes entre los que se destacan: daño directo por angiotensina II, lesión inducida por hipoxia, daño microvascular miocárdico y síndrome de respuesta inflamatoria sistémica. Conclusiones: Los diferentes mecanismos por los cuales el virus SARS-CoV-2 produce daño miocárdico, hacen que los pacientes con la COVID-19 tengan más probabilidades de sufrir una lesión miocárdica. Las manifestaciones clínicas en pacientes con la COVID-19 como miocarditis, insuficiencia cardíaca, arritmias cardíacas, síndrome coronario agudo y derrame pericárdico, son más comunes en pacientes con antecedentes de enfermedad cardiovascular que desfavorecen su pronóstico(AU)


Introduction: Coronavirus disease has caused myocardial damage. This reality has impacted medical sciences and it has been the subject of research. Objective: To show, through the results of recent research, the mechanisms by which the SARS-CoV-2 virus produces myocardial damage in patients affected by COVID-19, and its influence on short- and long-term prognosis. Methods: A bibliographic search was carried out of the updated medical literature on the topic published in English and Spanish, indexed in several databases from May 2019 to May 2022. One hundred ninety-eight articles were included in the review, only 78 met the selection criteria. Those that did not contain precise information regarding myocardial damage caused by SARS-CoV-2 were excluded. Results: Several mechanisms have been described as probable triggers, standing out direct damage by angiotensin II, hypoxia-induced injury, myocardial microvascular damage and systemic inflammatory response syndrome. Conclusions: The different mechanisms by which SARS-CoV-2 virus produces myocardial damage make COVID-19 patients more likely to suffer myocardial injury. Clinical manifestations in COVID-19 patients such as myocarditis, heart failure, cardiac arrhythmias, acute coronary syndrome and pericardial effusion are more common in patients with history of cardiovascular disease, which do not favors their prognosis(AU)

Introducción: el síndrome cardiorrenal es una patología dada por la disfunción en la interdependencia de estos órganos por interacciones bidireccionales (agudas o crónicas), los cuales pueden afectar indistintamente la función renal o ventricular. Objetivo: presentar y justificar la enfermedad renal crónica como desencadenante de cuadros congestivos por falla cardiaca de novo. Presentación del caso: se reporta el caso de un paciente masculino de 69 años revascularizado percutáneamente hace tres años con múltiples comorbilidades que ingresa en el contexto de una falla cardiaca de novo, secundaria a su enfermedad renal crónica estadio V de base, en manejo con hemodiálisis y en quien se descartó enfermedad coronaria aguda y miocardiopatía infiltrativa. Se logró estabilizar la injuria renal y cardiaca dando egreso y continuando manejo ambulatorio de sus patologías, al llevar un control adecuado de las mismas con Nefrología y Cardiología. Discusión y conclusión: la enfermedad cardiovascular generada por antecedentes renales tiene una gran repercusión en la función ventricular izquierda, causando hipertrofia, lo que lleva a una congestión con posterior sobrecarga debido a la caída del filtrado glomerular y que resulta en la disminución de la fracción de eyección. La enfermedad renal crónica predispone a alteraciones en la función cardiaca, lo que aumenta el riesgo cardiovascular.


Background: Cardiorenal syndrome is a pathology caused by dysfunction in the interdependence of these organs due to bidirectional interactions (acute or chronic), which can affect either renal or ventricular function. Purpose: To present and justify chronic kidney disease as a trigger of congestive conditions due to de novo heart failure. Case presentation: We report the case of a 69-year-old male patient percutaneously revascularized 3 years ago with multiple comorbidities who was admitted in the context of de novo heart failure secondary to his stage V chronic kidney disease on hemodialysis, in whom acute coronary artery disease and infiltrative cardiomyopathy were ruled out. The renal and cardiac injury was stabilized and the patient was discharged and continued outpatient management of his pathologies with adequate control of the same with nephrology and cardiology. Discussion and conclusion: Cardiovascular disease generated by renal history has great repercussion in left ventricular function causing hypertrophy that leads to congestion with subsequent overload due to the fall of glomerular filtration resulting in a decrease of the ejection fraction. Chronic kidney disease predisposes to alterations in cardiac function increasing cardiovascular risk.

Background: Several indexes have been developed to define the risk attributable to lipid metabolism with a single value. The total cholesterol/high-density lipoprotein (TC/HDL-C) and low-density lipoprotein/high-density lipoprotein (LDL-C/HDL-C) ratios are the most used. The higher the value of these ratios, the greater the probability of cardiovascular events. Objective: To identify whether the TC/HDL-C and LDL-C/HDL-C ratios are early prognostic markers of mortality and major cardiovascular events in patients with ST-elevation acute coronary syndrome. Material and methods: 265 patients with ST-segment elevation acute coronary ischemic syndrome were included, divided into 4 groups according to the values of the atherogenic indices. Mortality and major cardiovascular events at 30-day follow-up were analyzed. Comparison of the groups was performed using the chi-squared test or ANOVA, depending on the case (p < 0.05). Results: The cut-off point for the TC/HDL-C index was 6.9 and for the LDL-C/HDL-C it was 2.7. The comparative analysis of groups showed that cardiovascular death and arrhythmia were higher in group 3 (p = 0.006 and p = 0.003, respectively). Conclusions: TC/HDL-C and LDL-C/HDL-C indexes can be used as prognostic markers of cardiovascular mortality in the first 30 days of follow-up.


Introducción: se han elaborado diferentes índices para definir el riesgo atribuible al metabolismo lipídico con un solo valor. Los coeficientes colesterol total/lipoproteínas de alta densidad (CT/C-HDL) y lipoproteínas de baja densidad/lipoproteínas de alta densidad (C-LDL/C-HDL) son los más utilizados. A mayor valor de estos cocientes, la probabilidad de eventos cardiovasculares es mayor. Objetivo: identificar si los índices CT/C-HDL y C-LDL/C-HDL son marcadores pronósticos tempranos de mortalidad y evento cardiovascular mayor en pacientes con síndrome isquémico coronario agudo con elevación del ST. Material y métodos: se incluyeron 265 pacientes con síndrome isquémico coronario agudo con elevación del segmento ST, divididos en 4 grupos según los valores de los índices aterogénicos. Se analizó la mortalidad y el evento cardiovascular mayor en los 30 días de seguimiento. Se identificó el punto de corte de cada índice mediante un análisis de curva ROC. La comparación de los grupos se hizo con chi cuadrada o ANOVA, según fuera el caso (p < 0.05). Resultados: el punto de corte para el índice CT/C-HDL fue de 6.9 y para el C-LDL/C-HDL de 2.7. El análisis comparativo de los grupos demostró que la muerte cardiovascular y arritmia fue mayor en el grupo 3 (p = 0.006 y p = 0.003, respectivamente). Conclusiones: los índices CT/C-HDL y C-LDL/C-HDL pueden ser utilizados como marcadores pronósticos de mortalidad cardiovascular en los primeros 30 días de seguimiento.

La isquemia miocárdica es un fenómeno secundario a la perfusión insuficiente del músculo cardíaco que en algunos casos puede ocurrir de forma aguda llevando a la necrosis celular y constituyendo el infarto agudo al miocardio (IAM). A pesar de que el diagnóstico de IAM es principalmente clínico, en ciertos casos en que no se sospeche de forma activa por presentar síntomas no típicos de isquemia miocárdica, el diagnóstico puede sugerirse por la Tomografía Computarizada (TC), que puede mostrar hallazgos sugerentes de IAM. A continuación, se comunica una serie de 4 casos clínicos con diagnóstico imagenológico incidental de IAM.


Myocardial ischemia is secondary to myocardial under perfusion. It can develop acutely leading to cell necrosis and myocardial infarction (AMI), or have a chronic course. Though the diagnosis of AMI is mainly clinical, in certain cases the symptoms may be atypical and the diagnosis can be suggested by images such as Computed Tomography (CT). Herein we report a series of 4 clinical cases with diagnosis of AMI following incidental CT imaging. There was an abdominal pain in 3 patients and a cervical pain in the remaining one. CT scan showed a hypodense myocardial image. The final diagnosis was confirmed by the appropriate laboratory and angiographic methods.

Objetivo Evaluar el efecto del movimiento de «rebote» simulado por ordenador en los índices de perfusión y función del ventrículo izquierdo en relación con 3determinantes principales del movimiento (duración, magnitud y tiempo) mediante la evaluación del efecto único y la interacción de estos atributos en un modelo estadístico. Métodos Para el estudio, se seleccionaron 29 exploraciones gated-SPECT de perfusión miocárdica y, a continuación, se simuló manualmente el patrón de movimiento de «rebote» con respecto a 3atributos principales del movimiento: duración (corta vs. larga), magnitud (2 vs. 4 píxeles) y tiempo (precoz vs. tardío), todo ello en la dirección vertical ascendente. Todas las imágenes SPECT se reconstruyeron y filtraron utilizando un método idéntico (algoritmo OSEM) y con los mismos parámetros. Los índices de perfusión y función miocárdica del ventrículo izquierdo se obtuvieron a partir del programa informático QGS del Cedars-Sinai en las imágenes originales y de movimiento simulado y se comparan entre sí. Se aplicaron pruebas ANOVA de medidas repetidas intrasujeto de 2y 3vías para evaluar el efecto principal de cada variable o atributo y la interacción entre ellas. Resultados Las puntuaciones sumadas aumentaron aproximadamente de manera exponencial desde «sin movimiento» hasta un rebote corto y, después, hasta un rebote largo. En el rebote largo de 4 píxeles, los defectos de perfusión fueron notables. Todas las comparaciones de la extensión del defecto y del déficit total de perfusión fueron estadísticamente significativas. La diferencia media entre los patrones de movimiento de rebote corto y «sin movimiento» fue pequeña, incluso en los movimientos de 4 píxeles (casi del 3% o menor). Por el contrario, la diferencia media entre los patrones de movimiento de rebote largo y «sin movimiento» fue superior al 5% (AU)

Case description: A 61-year-old male patient with uncontrolled rheumatoid arthritis presented acute coronary syndrome on three occasions, less than 48 hours after infliximab infusion. Clinical findings: He presented with ST-elevation myocardial infarction on two occasions and non-ST-elevation acute coronary syndrome on one, with the identification of multivessel coronary disease. Treatment and outcome: Coronary intervention was performed with thrombus aspiration, medicated stent implantation, medicated balloon angioplasty, discontinuation of infliximab, and modification and optimization of cardiovascular pharmacological management. Clinical relevance: Patients with rheumatoid arthritis have subclinical cardiovascular disease and increased cardiovascular risk. The evidence regarding the relationship between infliximab and ischemic heart disease is controversial. A wide clinical spectrum of cardiac involvement with infliximab infusion is found in case reports, ranging from stable angina to ST-segment elevation acute coronary syndrome. The pathophysiology is not elucidated, with hypotheses proposing plaque rupture, allergic reactions, and vasoconstriction as possible disease mechanisms. The direct association between infliximab infusion and acute coronary syndrome needs more clinical research to optimize the management and prognosis of patients presenting with this type of complication.


Descripción del caso: Paciente masculino de 61 años con artritis reumatoide no controlada, en manejo con infliximab, quién presentó en tres oportunidades síndrome coronario agudo menos de 48 horas posterior a la aplicación del medicamento. Hallazgos clínicos: Presentó infarto con elevación del ST en dos ocasiones y síndrome coronario agudo sin elevación del ST en una oportunidad, encontrándose enfermedad coronaria multivaso. Tratamiento y resultado: Se realizó intervención coronaria con tromboaspiración, implante de stents medicados y angioplastia con balón medicado, suspensión del infliximab y modificación y optimización de manejo farmacológico cardiovascular. Relevancia clínica: Los pacientes con artritis reumatoide tienen enfermedad cardiovascular subclínica y mayor riesgo cardiovascular. La evidencia respecto a la relación entre infliximab y cardiopatía isquémica es controversial. En reportes de caso se encuentra un amplio espectro clínico de compromiso cardíaco con la infusión de infliximab, que va desde la angina estable hasta el síndrome coronario agudo con elevación del segmento ST. La fisiopatología no está claramente dilucidada, con hipótesis que proponen la ruptura de placa, reacciones alérgicas y la vasoconstricción como posibles mecanismos de enfermedad. La asociación directa entre la infusión de infliximab y el síndrome coronario agudo necesita más investigación clínica con el fin de optimizar el manejo y pronóstico de los pacientes que presentan este tipo de complicaciones.

INTRODUCTION AND OBJECTIVES: Randomized controlled trials comparing stress cardiac magnetic resonance (CMR) and single-photon emission computed tomography (SPECT) suggest similar diagnostic accuracy for detecting obstructive coronary artery disease (CAD). There are few data on whether this remains true in routine clinical practice. The aim of this study was to assess clinical and angiographic characteristics of patients undergoing invasive coronary angiography (ICA) after stress CMR or SPECT, and to compare their positive predictive value with published results from the CE-MARC trial. METHODS: In this retrospective tertiary-center analysis, we included 429 patients undergoing ICA after a positive stress CMR or positive SPECT performed within the previous 12 months. Obstructive CAD was defined as any coronary artery stenosis ≥50% in a vessel compatible with the ischemic territory on stress testing. RESULTS: Of the total 429 patients, 356 (83%) were referred after a positive SPECT, and 73 (17%) after a positive stress CMR. Patients did not differ according to age, cardiovascular risk factors, previous revascularization or left ventricular dysfunction, but patients with SPECT were more frequently male (p=0.046). The prevalence of obstructive CAD was similar in patients with positive SPECT vs. positive stress CMR (76.1% vs. 80.8%, respectively, p=0.385). The positive predictive values of both techniques were similar to those reported in the CE-MARC trial. CONCLUSION: In this tertiary center analysis, stress CMR and SPECT showed similar positive predictive values, comparable to those reported in the CE-MARC trial. This finding supports the emerging adoption of CMR in clinical practice for the diagnosis and management of CAD.

METHODS: Twenty-nine gated myocardial perfusion SPECT scans were selected for the study and then, "bounce" motion pattern was simulated manually regarding three main attributes of motion including duration (short versus long), magnitude (2 versus 4 pixels) and time (early versus late), all in upward vertical direction. All SPECT images are reconstructed and filtered with an identical method (OSEM algorithm) and same parameters. Indices of LV myocardial perfusion and function are derived using QGS package of Cedars-Sinai software in original and simulated-motion images and are then compared with each other. Value of end-diastolic volume (EDV) and end-systolic volume (ESV) are consistently decreased based on duration (from short to long) and magnitude (from 2 to 4 pixels). Using Within-Subjects ANOVAs, in long bounce, main effect of magnitude and interaction of magnitude and time, but not time solely, were statistically significant. In 2-pixel magnitude, none of variables and their interaction were significant, but in 4-pixel magnitude, EF showed statistical significance with duration. CONCLUSION: The perfusion parameters are to a higher extent involved by motion particularly in long bounce with a 4-pixel displacement. In short bounce, the effect is negligible, and therefore, no need to repeat the scan. Parameters of function are much less vulnerable to be affected by motion. Thus, contrary to current recommendations, there may be less need to repeat the scan in short 2-pixel bounce.

Introducción: la cardiopatía isquémica fue la primera causa de muerte en México en el año 2020. Su prevalencia aumenta con la edad y es superior en los hombres que en las mujeres; se presenta mayormente en forma de infarto en edades entre 45 y 94 años. Objetivo: describir el caso de un paciente sometido a revascularización aorto-coronaria por cardiopatía isquémica con enfoque del proceso de atención de enfermería (PAE). Metodología: estudio de caso observacional y descriptivo con aplicación del PAE en el perioperatorio de un hombre de 50 años con cardiopatía isquémica crónica, enfermedad multivascular e hipertensión sistémica controlada de 6 años desde que inició, en un hospital público de tercer nivel en Mérida, Yucatán, México. Resultados: se demostró que si se aplica el PAE hay menor riesgo de shock hipovolémico ocasionado por sangrado activo y disminución de riesgo de infección del sitio de herida quirúrgica, evidenciado por el procedimiento quirúrgico extenso. Conclusiones: la metodología del PAE como método científico facilita innovaciones dentro de los cuidados enfermeros, además de las diferentes alternativas en las acciones a seguir para el tratamiento del paciente quirúrgico cardiovascular. También proporciona un método informativo para la atención de cuidados, desarrolla una autonomía para la enfermería y fomenta la consideración como profesional de salud.


Introduction: Ischemic heart disease was the leading cause of death in Mexico in 2020. Its prevalence increases with age and it is higher in men than in women; it is presented mostly as a heart attack between the ages of 45 and 94 years. Objective: To describe the case of a patient undergoing aorto-coronary revascularization for ischemic heart disease with a nursing care process (NCP) approach. Methodology: Observational and descriptive case study with application of NCP in the perioperative period of a 50-year-old man with chronic ischemic heart disease, multivessel disease and controlled systemic hypertension of 6 years since its onset, in a third level public hospital in Merida, Yucatan, Mexico. Results: It was demonstrated that by applying NCP there is a lower risk of hypovolemic shock caused by active bleeding and decreased risk of surgical wound site infection, evidenced by the extensive surgical procedure. Conclusions: The NCP methodology as a scientific method facilitates innovations within nursing care, in addition to the different alternatives in the actions to follow for the treatment of the cardiovascular surgical patient. It also provides an informative method for care, develops autonomy for nursing and promotes consideration as a health professional.
